Turning CSV Chaos Into a Real-Time Healthcare Data Intelligence Platform
Client: A large managed care organization running a multi-provider street medicine program across multiple counties.
The Challenge
The organization was managing a network of street medicine providers, each using a different EHR, serving thousands of unhoused members across a large metropolitan region. Every month, staff received raw CSV exports from each provider, then manually consolidated them into spreadsheets to produce program reports. The data was inconsistent, the process was unsustainable, and leadership had no reliable way to see how the program was actually performing.
At the same time, the program was operating under strict compliance requirements: providers had to document member contact every 14 days, complete standardized health screenings, track housing status and hospital readmissions, and submit detailed outcome reports to their funding body, all without a system capable of aggregating the data needed to do any of it.
What Synegen Built
Synegen designed and built a custom healthcare data intelligence platform that aggregates information by pulling EHR data from every participating provider via SFTP, normalizes it to a unified data model, then surfaces it through role-based dashboards and automated reports.
- Multi-EHR data pipeline pulling from disparate provider systems into a single platform, with automated data mapping and normalization.
- Provider dashboards showing individual performance against program compliance metrics: contact frequency, screening completion, referral follow-through.

The Result
The organization went from manual CSV reconciliation to a real-time program intelligence platform. Providers can now see their own compliance status without waiting for a monthly report. Program leadership can identify members approaching deadline thresholds, track referral pipelines, and produce funder-ready reports in minutes instead of days.
